Yesterday was one of those intense and stimulating days that remind you why you chose this profession. We performed four robotic radical prostatectomies using three different platforms: the Da Vinci Xi robot, the Da Vinci SP (Single Port)—both from Intuitive—and Medtronic’s Hugo RAS system. Three robots with the same goal: operating with minimal invasiveness and maximum precision.
We were joined by doctors from all over the world who attended as observers and fellows: two from Colombia, two from Tanzania, one from Argentina, and one from Mexico. Each with their own training, their questions, their way of viewing surgery… And that greatly enriched the day. Because beyond operating, we debated.
One of the big questions that came up was: Which is the best robotic platform? And honestly, I don’t think there’s a single answer. It depends on the type of surgery, the patient’s characteristics, the surgeon’s experience and style… Each system has its advantages.
And of course, that led to the next big question: Who is the best surgeon? Well, the same applies. It depends on the platform being used, the team around them, the patient in front of them, and the environment they work in.
In my case, I always say the same thing: “Javier Romero Otero is the best robotic surgeon he himself can possibly be.” For me, it’s crucial to operate with my team in my usual setting, trusting the experience we’ve built together and carefully selecting the right technology for each patient. This doesn’t mean I can’t operate outside my comfort zone, adapting to different environments, teams, and circumstances—but when all factors (people, technology, and shared experience) align in the same direction, the potential to achieve surgical excellence multiplies.
We live in thrilling times for urological surgery. Technology gives us incredible tools, but what truly makes the difference is the human team, the passion for what we do, and keeping our focus always on the patient.
